Participant Information

AM I ELIGIBLE TO PARTICIPATE?

If you are registered with one of our participating GP practices in the Staffordshire and Stoke-on-Trent district, you may be eligible to obtain a free myhealthkit.

Participation in this evaluation is open to those who meet the eligibility criteria described below. This evaluation is carried out remotely. This means you can participate from home, with a Diabetic Wellness Pack posted to you, and information emailed directly to you from our team. Questionnaires can be filled out online or if needed via phone call with one of our team.

The Process

Upon registering, you will receive an email, which you need to confirm to both validate your details, and register your details with the website to access restricted pages. Within the email will be a link to a secure area, which you will need to log into with the information you provide below, to further provide more personal details to start the survey.

Following a successful application, you will be able to access online, additional support information, such as videos. A Diabetic Wellness Pack will also be sent in the post, designed to fit through the majority of letterboxes. You will then receive follow-up call/s to review how the pack is performing, and suited to your needs.

WHY ARE WE DOING THIS EVALUATION?

Simple assistive devices can support caregivers and help people do daily activities on their own. These can also help people manage their long-term health. To encourage self-care, it is important to let people know there are many helpful products they can easily get. People with diabetes often have foot problems like sores and infections. Checking their feet regularly and exercising more can help avoid these issues.

Our past research shows that putting simple, easy-to-get assistive devices and tools together in kits can make people more aware of self-care. It may also help them exercise more and manage their weight. However, we want to actually measure improvement and follow up long-term use of these products.

PURPOSE OF THIS WORK

Those consenting to be part of this evaluation will be sent a box with a set of assistive devices to help with the management of diabetes. We will collect some information about you and your feedback on using the box. This information will help the NHS to develop plans for how to help people manage their own long-term conditions such as diabetes.

WHY SHOULD I SIGN UP?

No one is obliged to join the study and no one will receive a lesser standard of care by choosing not to join. All participants will be able to access their usual NHS care that they would normally expect to receive.

Those who do wish to join the study will be helping in the evaluation of these kits and their usefulness in empowering people to manage their long term conditions. By joining as a participant you will be a critical part of this effort.

WHO IS FUNDING THIS WORK?

The costs of the kit provided to you are funded by the Department of Health and Social Care and NHS England to support the adoption and acceleration of technologies that can enhance patient care. Through this program, the Staffordshire and Stoke on Trent ICS has received funding to distribute and evaluate the effectiveness of providing “myhealthkits” to diabetic patients in community settings.

WHO CAN JOIN THE STUDY?

This evaluation is open to anyone who is registered with the participating GP practices and meets the following criteria:

a. Adults living in a community setting, aged 18 -75 with a diagnosis of type 1 or type 2 diabetes mellitus.
b. Ability to comprehend study information and provide informed consent[NC1]
c. Stable medication regimen in the 3 months prior to enrolment
d. Most recent HbA1c between 6.5%-10% (indicating relatively controlled blood sugar) in the last 6 months? [NC2]
e. No significant diabetes-related complications (e.g. diabetic retinopathy, neuropathy, nephropathy). For example no ulcers, no laser treatment or kidney problems
f. No major comorbidities that could substantially impact quality of life (e.g. stage 4 or higher CHF, ESRD, active cancer treatment)
g. No major mental health disorders or cognitive deficits that could preclude survey completion [NC3]
h. Willingness to complete periodic surveys on quality of life and self-care activities

WHAT DOES THIS EVALUATION INVOLVE?

If interested, you will be asked some brief questions online to check you are suitable, and provide informed consent to take part. Once you complete this information, you will receive one of our myhealthkits.

Taking part in this evaluation will involve answering a few questions online while signing up and then a set of questions at week 6 after receiving the kit and at week 12 and week 18 and receiving a phone call from the evaluation team during weeks 6 and 18 to speak about your activities of daily living.

How to sign-up

You can participate from the comfort of your own home from anywhere in the UK. No face-to-face visits are required. Follow-up will be by answering questions online and/or telephone calls with the evaluation team.
